Notice to patients – Dr. Hubley & Dr. Chukwuogo are leaving HealthPoint – Madisonville Clinic. Their last days are last day practicing at this clinic will be December 31, 2022 (Hubley), and December 16, 2022 (Chukwuogo).

Effective December 16, 2022 (Chukwuogo) and December 31, 2022 (Hubley), will no longer be practicing at HealthPoint’s Madisonville Clinic.

It has been a great honor to care for you and your family over the years, and it is not easy for us to leave.
We are pleased to inform you that you can continue your care at HealthPoint’s Madisonville Clinic without interruption. Dr. Imelda Villanueva, a long-time Madisonville provider, will continue to lead the HealthPoint Madisonville team. You will have the opportunity to meet and establish a relationship with one of our highly skilled providers, two of whom are joining the HealthPoint team in the coming days:
- Dr. Imelda Villanueva
- Dr. Hugh Tadlock
- Michael Burrows, FNP
While we are sad to leave our patients, we know you are in capable hands and will welcome the new providers with the same warmth I experienced. HealthPoint is invested in the Madisonville community and committed to being the BEST place to receive primary care.
Should you choose to seek medical care from a provider outside the HealthPoint network, a copy of your medical records will be transferred to that provider or released to you or a person you designate at your request. You can scan the QR code at the bottom of this letter to request your records or ask your local clinic for a medical records release form. Once completed, return it to any of our clinics.

As a patient of HealthPoint Navasota, the following options are available to you:
- You may continue your care at the HealthPoint Navasota Clinic. Please call (936) 348-3418 if you need to schedule an appointment.
- If you want to see one of our doctors at a different clinic, please navigate back to our home page.
- If you are interested in seeing a doctor outside of our network of clinics, we will make your records available to any doctor of your choice. Please request a medical records release form and return it to any of our clinics so we can send your medical records to your new doctor. You have the right to see providers outside of our network, but we hope you will give us the opportunity to continue to serve you and your family.
