Announcement regarding HealthPoint – Trinity Clinic
Medical Records Request Form
We are writing today to inform you of the upcoming Trinity Clinic closure. It is with mixed emotions that we close our HealthPoint – Trinity Clinic effective 12/30/2022. While we will continue to provide medical care until 12/30/2022, we suggest you establish a relationship with another provider before this date to guarantee your access to care.
Your medical records are confidential and protected by federal privacy and security regulations. We will release a copy of your medical records to you, your new provider, or a person you designate only with your written request. You can scan the QR code at the bottom of this letter to request your records or ask your local clinic for a medical records release form. Once completed, return it to any of our clinics.
As a patient of HealthPoint, the following options are available to you:
- If you wish to see one of our providers at a different clinic, please refer to our website to find one near you: www.healthpoint-tx.com.
- If you are interested in seeing a provider outside of our network of clinics, fill out the form found below or request a copy from one of our clinics.
- You must supply the name, address, and phone number of your new provider.
PLEASE NOTE: Due to the Trinity Clinic closure after 12/30/2022, you may obtain a copy of your medical records by emailing: medicalrecords@healthpoint-tx.com or calling our Medical Records Department at: (979) 977-3010.
