Release of Information (ROI) requests for medical records and/or billing records submitted to HealthPoint are now processed by HealthMark Group, effective September 14, 2026.

HealthMark Group is a trusted third-party business associate that provides a dedicated team focused solely on releasing HealthPoint’s medical and billing records in accordance with regulatory requirements.
All questions regarding ROI request status and processing can be made to the HealthMark Group directly.
The quickest way to access your medical records is by requesting through the HealthMark Group.
However, ROI requests may alternatively be submitted by printing and completing the Patient Initiated Release of Medical Information form, then submitted directly to HealthPoint through the following methods:
